Applications Forms

The following are forms that those applying for positions within the 1st Special Operation Force Support Squadron's facilities may need as part of their application:

— "OF-612" and "OF-612 Supplement" are part of the application package for federal employment and must be submitted for each position applied for, résumés may be attached

— "OF-306, Declaration for Federal Employment" MUST accompany the application package

— "Acknowledge of Rights and Consent to Release Records" form

Anyone applying for positions working directly with children must fill out "Supplemental Child or Youth Appendix B and C"
