This web-based incentive program will collect personal information that will only be used to notify the child when they have achieved a level, or won a prize, or to provide information regarding FitFactor events. This data will be securely stored and not shared or provided to any third parties.

The personal information collected will include: child's name, birth date, gender, school grade, home phone number and email address. Also collected will be Base where child resides, Branch of Service and parent/guardian's name. You have the right to revoke this consent at any time.
